1. (a) Licensees are responsible for compliance with the health and safety standards of this chapter.
2. (b) A licensee shall be restricted to working in a cosmetology establishment licensed under this chapter.
3. (c) A licensee who leases space as an independent contractor on the premises of a cosmetology establishment to engage in any practice of cosmetology authorized under the Act must obtain a booth rental permit.
4. (d) Specialty certificate holders may only perform the practice authorized by the specialty certificate.
5. (e) Individual licenses and booth rental (independent contractor) licenses must be posted at the licensee's work station in the public view.
6. (f) A current photograph of the licensee approximately 1 1/2 inches by 1 1/2 inches shall be attached to the front of the license, certificate or permit.
7. (g) Licensees shall notify the department in writing of any name change within 30 days of the change.
8. (h) Licensees must notify the department not later than thirty (30) days following any change of address. The department may send all notices on other information required by applicable laws and rules to any licensee's last known mailing address on file with the department.
9. (i) Cosmetology services may be performed on incapacitated or deceased persons provided that the appointment is made through the salon. Licensees must have their license in their possession while performing the service.
10. (j) Licensees shall wear clean top and bottom outer garments and footwear while performing services authorized under the Act. Outer garments include tee shirts, blouses, sweaters, dresses, smocks, pants, jeans, shorts, and other similar clothing and does not include lingerie or see-through fabric.
