1. (a) On or after March 1, 2006, no person may operate as an administrator for a provider or offer to act as an administrator for a provider operating in this state without first registering with the department.
2. (b) A registration is valid for one year from the date issued and must be renewed annually.
3. (c) Both initial and renewal applications for registration must provide the department with all information required, on a form prescribed by the executive director.
4. (d) Both initial and renewal applications must be accompanied by the required fee(s).
5. (e) Falsification of information on an application is cause for denial and/or revocation of the registration.
