1. (a) The owner or operator of a boiler in this state must submit the following to the department within thirty (30) days after completion of a boiler installation.

   1. (1) A boiler installation report; and
   2. (2) A manufacturer's data report for the boiler.
2. (b) The boiler shall not be test-fired or operated prior to the required first inspection except as allowed under subsection (c).

3. (c) Temporary Operating Permit.

   1. (1) The owner or operator may request a Temporary Operating Permit on a department-approved form.
   2. (2) The owner or operator must pay the applicable fee required under §65.300.
   3. (3) Upon approval of the Temporary Operating Permit from the department, the boiler may be operated prior to the required initial inspection for up to thirty (30) days.
