(a) Provider application fee per occupation: $250.
(b) Provider renewal application fee per occupation: $250.
(c) Course-approval fee per occupation: $100.
(d) Issuance of a revised or duplicate registration: $25.
(e) All fees paid to the department are non-refundable.
