1. (a) All fees paid to the department are non-refundable.
2. (b) The two-year initial licensing fee for a LHR facility license is $900.
3. (c) The two-year renewal fee for an LHR facility license is $750.

4. (d) The two-year initial licensure fees and two-year renewal fees for individual LHR certifications are as follows:

   1. (1) LHR professional--$150;
   2. (2) Senior LHR technician--$100;
   3. (3) LHR technician--$70; and
   4. (4) LHR apprentice-in-training--$50.
5. (e) A duplicate/replacement fee for a license or certification issued under this chapter is $25.
6. (f) Late renewal fees for licenses and certifications issued under this chapter are provided under §60.83.
7. (g) A dishonored payment fee is the fee prescribed under §60.82.
8. (h) The fee for a criminal history evaluation letter is the fee prescribed under §60.42.
