1. (a) Fees paid to the department are nonrefundable.

2. (b) Hearing Instrument Fitter and Dispenser License:

   1. (1) Initial application fee (includes two-year initial license)--$205;
   2. (2) Renewal application fee (for two-year license)--$205;

3. (c) Apprentice Permit:

   1. (1) Initial application fee (includes one-year permit)--$205;
   2. (2) Extension fee (one-year term, may only be extended once)--$25;

4. (d) Temporary Training Permit:

   1. (1) Initial application fee (includes one-year permit)--$205;
   2. (2) Extension fee (one-year term, may only be extended once)--$25;
5. (e) The fees for the written test, the practical test, and the jurisprudence test are set by and payable to the department's designees.

6. (f) Continuing Education Provider Registration:

   1. (1) Initial application fee--$200 before May 1, 2025; $400 on or after May 1, 2025.
   2. (2) Renewal application fee--$200 before May 1, 2025; $400 on or after May 1, 2025.
7. (g) A duplicate/replacement fee for a license or permit issued under this chapter is $25.
8. (h) Late renewal fees for licenses and permits issued under this chapter are provided under §60.83 of this title (relating to Late Renewal Fees).
9. (i) A dishonored/returned check or payment fee is the fee prescribed under §60.82 of this title (relating to Dishonored Payment Device).
10. (j) The fee for a criminal history evaluation letter is the fee prescribed under §60.42 of this title (relating to Criminal History Evaluation Letters).
