1. (a) A temporary training permit is valid for one year. The department may extend the temporary training permit for an additional period not to exceed one year. A temporary training permit may not be extended more than once.

2. (b) To extend a temporary training permit, the temporary training permit holder must:

   1. (1) submit an extension request on a department-approved form;
   2. (2) submit a new or renewed supervision agreement for the extension period; and
   3. (3) pay the permit extension fee required under §112.110.
3. (c) To extend the permit, a temporary training permit holder must successfully pass a criminal history background check pursuant to Occupations Code, Chapters 51 and 53, and the department's criminal conviction guidelines.
4. (d) A person whose permit has expired shall not practice the fitting and dispensing of hearing instruments.
5. (e) A person who has been issued a second temporary training permit under §112.50 may extend the second temporary training permit once in accordance with this section.
