1. (a) Unless otherwise indicated, an applicant must submit all required information and documentation of credentials on department-approved forms.
2. (b) An applicant must complete all permit requirements within one year from the date the application was submitted. After that year an applicant will be required to submit a new application and all required materials in addition to paying a new application fee.

3. (c) An applicant for a temporary training permit must:

   1. (1) have never taken the examination administered under this chapter;
   2. (2) provide documentation that the applicant is at least 18 years of age;
   3. (3) submit a completed application on a department-approved form;

   4. (4) submit one of the following education records:

      1. (A) an official diploma or official transcript indicating graduation from an accredited high school;
      2. (B) a certificate of high school equivalency issued by the appropriate education agency; or
      3. (C) an official diploma or official transcripts from an accredited college or university indicating a college degree was obtained;
   5. (5) submit the supervisor statement required under Texas Occupations Code §402.252, on a department-approved form; and
   6. (6) pay the temporary training permit fee required under §112.110.
4. (d) An applicant for a temporary training permit must successfully pass a criminal history background check.
