- (a) All prescriptions for liquor written by the holders of a physician's permit shall be written upon a form prescribed by the administrator (liquor prescription). Said forms for prescriptions shall be printed in duplicate in bound book form and shall bear serial numbers running in consecutive order for each form printed. The commission shall have such prescription forms printed and furnish same to holders of physician's permits upon request, as hereinafter provided.
- (b) Each holder of a physician's permit in writing a prescription for liquor shall make an exact duplicate of the original thereof, deliver the original to the patient, and retain the duplicate of said prescription in the bound book furnished him, which duplicate shall be kept by the holder of said physician's permit for a period of two years.
- (c) When the holder of a physician's permit desires prescription blanks upon which to write liquor prescriptions, he shall make application to the administrator on forms prescribed by the administrator for prescription blanks upon which to write liquor prescriptions. In such form said holder of a physician's permit shall make an order for 100 prescriptions forms and in such order he shall designate the beginning date of the 90-day period, for which period he is restricted by law to the writing of not more than 100 prescriptions for liquor. He shall also give the number and serial number of any unused prescription forms he may have and details in regard to any voided, mutilated or destroyed prescriptions forms. Forms for such requisitions or orders for prescription blanks shall be furnished holders of physician's permits by the commission upon request from such holders of physician's permit. Prescription forms shall not be issued to any physician in excess of 100 forms for any designated 90-day period. No holder of a physician's permit shall write a prescription for liquor on any form other than one issued to him by the commission.
- (d) Every holder of a physician's permit shall require the person for whom he writes a liquor prescription to endorse and sign his name on the back of such prescription in the presence of the prescribing physician, and it shall be a violation of this section for the holder of a medicinal permit to fill a liquor prescription until the person for whom the prescription is written has endorsed and signed his name on the back of such prescription.
