(a) Upon each sale or delivery of liquor by the holder of a package store permit of more than three gallons of liquor, an invoice shall be prepared in duplicate originals, both originals of which shall fully reflect thereon the following information:
- (1) name and address and permit number of the package store at which, or from which, the sale is made;
- (2) month, date, and year of sale;
- (3) the name and city and state address of the person to whom the liquor is sold;
- (4) the license number of any motor vehicle into which such liquor is loaded on the premises of the package store permittee;
- (5) the time of day in which such liquor is sold or delivered to the purchaser or receiver;
- (6) The quantity and selling price in cases or gallonage of each brand and type of liquor sold;
- (7) the signature of the person making the sale or delivery.
- (b) It shall be unlawful for the holder of a package store permit to fail to prepare an invoice as herein required upon each sale of more than three gallons of liquor, or to use, issue or prepare such invoice in a manner failing to meet all requirements of this section, or to sell or deliver such liquor without signing his true name to the invoice, herein required, or to knowingly enter or show upon such invoice any untrue fact, figure, name, address, or circumstance, or to fail or refuse to keep at the place of business where such sale was made for a period of two years each invoice issued in compliance with this section.
