An emergency order issued under this subchapter shall contain at least the following:

1. (1) the name and address of the license or permit holder and information sufficient to identify the premises affected by the order;
2. (2) a brief statement of fact supporting the issuance of the order;
3. (3) a determination that the continued operation of a permitted or licensed business would constitute a continuing threat to the public welfare; and
4. (4) a statement of the term of the suspension, including the date on which it begins and ends.
