If an LNG Form 2004 decal or letter of authority on a unit currently registered with LP-Gas Operations is destroyed, lost, or damaged, the operator of that vehicle shall obtain a replacement decal or letter of authority by filing LNG Form 2018B and a $50 replacement fee with LP-Gas Operations.
