Registered borrowers of the Texas State Library, Program for the Blind and Physically Handicapped should notify the library if any of the following circumstances occur:

1. (1) the borrower moves to a different address or gets a different telephone number;
2. (2) the borrower desires to cancel service permanently or to place service on temporary hold for vacation, illness, or other reasons;
3. (3) the borrower moves temporarily or permanently to a location outside the State of Texas;
4. (4) the borrower ceases to be eligible for service.
