A termination statement shall contain the following information for the purpose of maintaining the UCC information management system.

1. (1) Identification of the document as a termination statement.
2. (2) Identification of the original financing statement (as defined in §95.101(8) of this title (relating to Definitions)).
3. (3) The name of the secured party(ies) of record whose interest is terminated.
