Statement of Officer Form

- (a) A statement of officer form containing the language required by Article XVI, §1 of the Texas Constitution is available on the secretary of state web site at http://www.sos.state.tx.us/statdoc/statforms.shtml or may be obtained by writing the Statutory Documents Section, Office of the Secretary of State, P.O. Box 13550, Austin, Texas 78711-3550. See Form 2201.
(b) A statement of officer must provide:
- (1) the language required by Article XVI, §1(b) of the Texas Constitution;
- (2) the specific office to which the officer has been elected or appointed; and
- (3) the typed or printed name and signature of the officer.
- (c) In addition to other accepted methods of delivery, the statement of officer may be submitted to the secretary of state by facsimile.
