The application for an organizer's card shall be on a form to be provided by the Office of the Secretary of State. The Statutory Documents Section of the Office of the Secretary of State hereby adopts by reference the following form, "Application for Labor Organizer Card." All persons required to file an application shall use this form. Copies may be obtained by contacting the Office of the Secretary of State, Statutory Documents Section, P.O. Box 12887, Austin, Texas 78711-2887.
