Forms shall be provided by the Office of the Secretary of State for the purposes of complying with the Act and this chapter. The forms are hereby adopted by reference and may be obtained from the Office of the Secretary of State, Statutory Documents Section, P.O. Box 12887, Austin, Texas 78711-2887.
