Evaluation Requirements of the Program

- (a) The funded applicant will be required to furnish to the department an annual fiscal report and annual program report.
(b) The annual report will include:
- (1) the number of reports received;
- (2) the number of persons counseled and number of counseling hours;
- (3) the number of persons referred for other definitive health care; and
- (4) the number of public education programs given and the number of persons participating in each.
