All bills for medical, chiropractic, psychiatric, and psychological care, and all bills of duly licensed practitioners providing remedial treatment to the victim or claimant for the condition resulting from the crime, must be itemized and submitted on a form approved by the OAG. If a collateral source is available, the explanation of benefits (EOB) showing the utilization of collateral sources must be attached.
