If possible, applicants should submit a signed Comprehensive Certification and Assurances Form with the grant application. Otherwise, the form must be submitted within the period for acceptance or rejection of a grant as provided in §60.102(c). The form includes the following certifications:

1. (1) Equal Employment Opportunity Program Certification;
2. (2) Certification Regarding Lobbying;
3. (3) Nonprocurement Debarment Certification;
4. (4) Drug-Free Workplace Certification;
5. (5) Audit Certification;
6. (6) UGMS Certification;
7. (7) Certified Assurances; and
8. (8) Other certifications deemed necessary by the OAG.
