1. (a) Fee-related negotiations. Each application submitted to the OAG that proposes a fee-related negotiation must be accompanied by a $4000 fee. In addition, each contract (including contract renewals and modifications) submitted to the OAG which is the product of a fee-related negotiation must be accompanied by a $1000 fee.
2. (b) Non-fee-related negotiations. Each application submitted to the OAG that proposes only non-fee-related negotiations must be accompanied by a $2000 fee. In addition, each contract (including contract renewals and modifications) submitted to the OAG which is the product of only non-fee-related negotiations must be accompanied by a $500 fee.
3. (c) Physicians' representative. In addition to the fees listed above, for each application submitted to the OAG, the representative shall pay to the TDI a $500 fee to act as a representative.
4. (d) Payment of fees. The fees required by subsections (a) and (b) of this section shall be by cashier's check or money order, made payable to the Office of the Attorney General. The fee required by subsection (c) of this section shall be by cashier's check or money order, made payable to the Texas Department of Insurance. All fees are nonrefundable, except as provided in §58.11(f) and §58.25 of this chapter (relating to withdrawal of application).
