1. (a) The executive director shall produce and certify all forms used to disclose information to be reported under these rules or by a law administered and enforced by the commission.
2. (b) A form prescribed by the executive director under subsection (a) of this section shall include all information required by these rules and by other applicable law.
3. (c) Each form prescribed under this section shall be made available by the executive director for use by those required to file a report.
4. (d) Twenty days before a form is adopted under this section, the executive director shall send a copy of each such form to each member of the commission.
