1. (a) This section does not apply to a committee that has not appointed a campaign treasurer in accordance with §20.303(b) of this chapter (relating to Appointment of Campaign Treasurer).

2. (b) The notice required by §254.128 of the Election Code shall be in writing and shall include:

   1. (1) the full name of the committee;
   2. (2) the address of the committee;
   3. (3) the full name of the committee's campaign treasurer;
   4. (4) the address of the committee's campaign treasurer;
   5. (5) a statement that indicates that the committee is a political action committee; and
   6. (6) a statement that the committee has accepted political contributions or has made political expenditures on behalf of the candidate or officeholder.
