In addition to the information required by §254.202 of the Election Code, an annual report of unexpended contributions shall include the following information:

1. (1) for each payment made by the candidate from unexpended political contributions, unexpended interest or other income earned from political contributions, or assets purchased with political contributions or interest or other income earned from political contributions during the previous year:

   1. (A) the full name of each person to whom a payment was made;
   2. (B) the address of each person to whom a payment was made;
   3. (C) the date of each payment;
   4. (D) the nature of the goods or services for which the payment was made; and
   5. (E) the amount of each payment;
2. (2) the full name of each person to whom a payment from unexpended political contributions, unexpended interest or other income earned from political contributions, or assets purchased with political contributions or interest or other income earned from political contributions was made.
