Each report must include for each political expenditure of any amount made out of personal funds for which reimbursement from political contributions is intended:

1. (1) the full name of the person to whom each expenditure was made;
2. (2) the address of the person to whom the expenditure was made;
3. (3) the date of the expenditure;
4. (4) the purpose of the expenditure;
5. (5) a declaration that the expenditure was made out of personal funds;
6. (6) a declaration that reimbursement from political contributions is intended; and
7. (7) the amount of the expenditure.
