1. (a) Each report must include for each person making a loan or loans to the candidate for campaign purposes if the total amount loaned by the person during the reporting period is more than the threshold amount:

   1. (1) the full name of the person or financial institution making the loan;
   2. (2) the address of the person or financial institution making the loan;
   3. (3) the amount of the loan;
   4. (4) the date of the loan;
   5. (5) the interest rate;
   6. (6) the maturity date;
   7. (7) the collateral for the loan, if any; and

   8. (8) if the loan has guarantors:

      1. (A) the full name of each guarantor;
      2. (B) the address of each guarantor;
      3. (C) the principal occupation of each guarantor;
      4. (D) the name of the employer of each guarantor; and
      5. (E) the amount guaranteed by each guarantor.
2. (b) the total amount of loans accepted during the period for the threshold amount and less from persons other than financial institutions engaged in the business of making loans for more than one year, except for a loan reported under subsection (a) of this section.
