Each report must include for each person from whom the candidate accepted a pledge or pledges to provide more than the threshold amount in money or goods or services worth more than the threshold amount:

1. (1) the full name of the person making the pledge;
2. (2) the address of the person making the pledge;
3. (3) the amount of each pledge;
4. (4) the date each pledge was accepted; and
5. (5) a description of any goods or services pledged; and
6. (6) the total of all pledges accepted during the period for the threshold amount and less from a person.
