1. (a) Personal care services may be provided in an individual or group setting.

2. (b) Personal care services may be authorized for the following place(s) of service:

   1. (1) The beneficiary's home;
   2. (2) The home of the primary or alternate care giver;
   3. (3) The beneficiary's school;
   4. (4) The beneficiary's day care facility; or
   5. (5) Any community setting in which the beneficiary is located.
3. (c) Personal care services may not be authorized in hospitals, nursing facilities, intermediate care facilities for the mentally retarded, or institutions for mental disease.
