1. (a) A crime stoppers organization that is certified by the Crime Stoppers Advisory Council must submit to the director of the Crime Stoppers Advisory Council a Crime Stoppers Program Information Update Form no later than January 31 of each calendar year.

2. (b) A Crime Stoppers Program Information Update Form must include the following information:

   1. (1) The name, mailing address, email address, and telephone number of the crime stoppers organization, and the internet address of any website operated by the organization;
   2. (2) The name, mailing address, email address, telephone number, occupation, and board position of each member of the organization's governing board; and
   3. (3) The name, mailing address, email address, and telephone number of each of the organization's law enforcement/civilian coordinators.
