1. (a) The committee shall issue a written opinion containing each of its determinations and a separate statement of the committee's findings of fact.

2. (b) The ICF-IID shall send a copy of the committee's opinion to:

   1. (1) each person notified under Section 597.045; and
   2. (2) the department.
