1. (a) An individual who offers to provide consulting services to a state agency and who has been employed by that agency or by another agency at any time during the two years preceding the making of the offer shall disclose in the offer:

   1. (1) the nature of the previous employment with the agency or the other agency;
   2. (2) the date the employment was terminated; and
   3. (3) the annual rate of compensation for the employment at the time of its termination.
