A license, permit, or tag issued pursuant to this title may be replaced only upon affidavit from the licensee that the original was lost or destroyed and upon payment of the fee:

1. (1) for a duplicate license or permit the fee is three dollars, one dollar of which the issuing sales vendor may retain;
2. (2) for a duplicate disability or lifetime license issued by the department, there is no fee;
3. (3) for a replacement nongame fish tag, the fee is one dollar per tag for residents and five dollars per tag for nonresidents;
4. (4) for a duplicate individual antlerless deer tag, the fee is one dollar per tag.
