(A)

1. (1) Individual hospitals shall collect data on hospital acquired infection rates for the specific clinical procedures as recommended by the advisory committee and defined by the department, including the following categories:

   1. (a) surgical site infections;
   2. (b) ventilator associated pneumonia;
   3. (c) central line related bloodstream infections; and
   4. (d) other categories as provided under subsection (D).
