(1) submit a written application in the form prescribed by the Board of Pharmacy at least forty-five days before the opening date of the facility;
(2) pass an inspection conducted by inspectors of the Board of Pharmacy; and
(3) pay all appropriate fees.
(B) Applicants for permits to distribute, manufacture, sell, purchase, or produce drugs or devices within this State shall file with the Board of Pharmacy a verified application containing such information as the board requires of the applicant relative to the qualifications for a permit holder.
(C) Permits issued by the board pursuant to this chapter are not transferable or assignable.
