1. (A) To apply for licensure as a massage therapy establishment or a sole practitioner establishment, an applicant must submit:

   1. (1) a completed application on a form prescribed by the board, accompanied by all applicable fees for each massage therapy establishment or sole practitioner establishment location;
   2. (2) the name, address, and telephone number of each owner of the massage therapy establishment or sole practitioner establishment; and
   3. (3) the physical address, mailing address, and telephone number of the premises of the massage therapy establishment or sole practitioner establishment.
2. (B) A massage therapy establishment or sole practitioner establishment must be organized or registered under applicable South Carolina law as an entity, as defined in this chapter.
3. (C) Upon the board's receipt and approval of the information required by this section, the department shall conduct a pre-licensing inspection of the proposed establishment to determine whether the establishment may be licensed as a massage therapy establishment or a sole practitioner establishment. The department shall provide its determination on a form approved by the board.
