1. (1) The employer shall keep and make available for inspection by the Commissioner of the Bureau of Labor and Industries or representatives of the commissioner the following documents:

   1. (a) A true copy of the notice delivered to employees pursuant to these rules;
   2. (b) If the notice was mailed, the certificate of mailing for each employee to whom a notice was mailed;
   3. (c) If the notice was distributed in person, a written acknowledgement, signed by the employee, that the employee received the notice.
2. (2) The documents required to be kept in section (1) of this rule shall be maintained for a period of two years from the date the notice was delivered.
