1. (1) Application for registration shall be submitted through the Nationwide Multistate Licensing System and must be accompanied by the required fee.

2. (2) Applications shall include:

   1. (a) The information required by ORS 697.031(1)(a) through (e);
   2. (b) The name of the collection agency trust account, account number and financial institution name and address;
   3. (c) Telephone number of the collection agency business;
   4. (d) Name of the collection agency business manager; and
   5. (e) Address of all business locations.
