1. (1) A notary public whose notarial journal is lost, misplaced, destroyed or otherwise unusable may file a written notice, under oath or affirmation, with the Secretary of State.

2. (2) The notice shall set forth:

   1. (a) A statement of whether the notarial journal is lost, misplaced, destroyed or in some other manner made unusable;
   2. (b) An explanation of how the notarial journal became unusable.
   3. (c) The date the notary public discovered that the notarial journal was unusable.
   4. (d) A statement that the notary public does not possess the notarial journal and does not know who possesses it or where it is located.
