Application for examination, certification, or license or renewal of license shall be made to the Commissioner in writing on forms furnished by the Commissioner and each application shall be accompanied by a fee of Twenty-five Dollars ($25.00). Applicants for renewal may also be required to submit proof of compliance with continuing education requirements established by the Board.
