Each surplus line broker shall keep in his office in this state a full and true record of each surplus line contract procured by him, and such record may be examined at any time within three (3) years thereafter by the Insurance Commissioner or the Insurance Board. The record shall include such of the following items as are applicable:

1. 1. Name and address of the insurer,
2. 2. Name and address of the insured,
3. 3. Amount of insurance,
4. 4. Gross premium charged,
5. 5. Return premium paid, if any,
6. 6. Rate of premium charged on the several items of coverage,
7. 7. Effective date of the contract and the terms thereof, and
8. 8. Brief general description of the risks insured against and the property insured.
