1. A. The court shall ensure that the following information accompanies any deprived child placed outside the child's home as soon as the information becomes available:

   1. 1. Demographic information;
   2. 2. Strengths, needs and general behavior of the child;
   3. 3. Circumstances which necessitated placement;
   4. 4. Type of custody and previous placement;
   5. 5. Pertinent family information including, but not limited to, the names of family members who are and who are not, by court order, allowed to visit the child and the child's relationship to the family which may affect placement;
   6. 6. Known and important life experiences and relationships which may significantly affect the child's feelings, behavior, attitudes or adjustment;
   7. 7. Whether the child has third-party insurance coverage which may be available to the child;
   8. 8. Education history to include present grade placement, last school attended, and special strengths and weaknesses. The Department of Human Services shall also assist the foster parents in getting the foster child's school records and gaining school admission; and

   9. 9. Known or available medical history including, but not limited to:

      1. a. allergies,
      2. b. immunizations,
      3. c. childhood diseases,
      4. d. physical handicaps,
      5. e. psycho-social information, and
      6. f. the name of the child's last doctor, if known.

2. B. When the Department places a child in out-of-home care, the Department shall provide the placement providers with sufficient medical information to enable the placement providers to care for the child safely and appropriately. Such medical information shall include, but not be limited to:

   1. 1. Any medical or psychological conditions;
   2. 2. Diseases, illnesses, accidents, allergies, and congenital defects;
   3. 3. The child's Medicaid card or information on any other third-party insurer, if any; and
   4. 4. Immunization history.
