If the Department grants a variance as specified in OAC 310:257-15-3.1, or a HACCP plan is otherwise required as specified under OAC 310:257-15-8, the license holder shall:

1. (1) Comply with the HACCP plans and procedures that are submitted as specified under OAC 310:257-15-9 and approved as a basis for the variance, and

2. (2) Maintain and provide to the regulatory authority, upon request, records specified under OAC 310:257-15-9(5) and 6(B) that demonstrate that the following are routinely employed;

   1. (A) Procedures for monitoring critical control points,
   2. (B) Monitoring of the critical control points,
   3. (C) Verification of the effectiveness of an operation or process, and
   4. (D) Necessary corrective actions if there is failure at a critical control point.
