A person desiring to operate a food establishment shall submit to the regulatory authority a written application for a license on a form provided by the Department. The application will include at a minimum:

1. (1) The name, mailing address, telephone number, e-mail address, and signature of the person applying for the license and the name, mailing address, and physical location of the food establishment;
2. (2) Information specifying whether the food establishment is owned by an association, corporation, individual, partnership, or other legal entity.
