Except as provided in section 4715.73 of the Revised Code, a mobile dental facility operator or the operator's representative shall notify the state dental board and all treatment venues not later than fourteen days after either of the following occurs:

1. (A) There is a change in the address or telephone number of the operator.
2. (B) The mobile dental facility ceases to operate. The notice must be provided in writing.
