Ohio Rev. Code Ann. § 4506.08 – Application and driving record information fees

(A)
1. (1) Each application for a commercial driver's license temporary instruction permit shall be accompanied by a fee of ten dollars.
2. (2) Each application for a waiver for farm-related service industries shall be accompanied by a fee of twenty-five dollars.
3. (3) Except as provided in division (A)(4) of this section, each application for a commercial driver's license, restricted commercial driver's license, or renewal of such a license shall be accompanied by the following fee, as applicable:
  1. (a) A fee of twenty-five dollars if the license will expire on the licensee's twenty-first birthday;
  2. (b) A fee of twenty-five dollars if the license will expire on the licensee's birthday four years after the date of issuance;
  3. (c) A fee of forty-nine dollars and fifty cents if the license will expire on the licensee's birthday eight years after the date of issuance.
4. (4) Each application for a commercial driver's license or restricted commercial driver's license received pursuant to division (A)(3) of section 4506.14 of the Revised Code shall be accompanied by the following fee, as applicable:
  1. (a) A fee of eighteen dollars and seventy-five cents if the license will expire on the licensee's birthday three years after the date of issuance;
  2. (b) A fee of twelve dollars and fifty cents if the license will expire on the licensee's birthday two years after the date of issuance;
  3. (c) A fee of six dollars and twenty-five cents if the license will expire on the licensee's birthday one year after the date of issuance.
5. (5) Each application for a duplicate or reprint commercial driver's license shall be accompanied by a fee of ten dollars.
(B)
1. (1) The registrar of motor vehicles or deputy registrar may collect and retain an additional fee equal to the amount established under section 4503.038 of the Revised Code for each application received by the registrar or deputy registrar for any of the following:
  1. (a) A commercial driver's license temporary instruction permit;
  2. (b) A commercial driver's license or a restricted commercial driver's license that will expire on the licensee's twenty-first birthday;
  3. (c) A commercial driver's license, a restricted commercial driver's license, or a renewal of such a license that will expire on the licensee's birthday four years after the date of issuance;
  4. (d) A commercial driver's license or restricted commercial driver's license issued pursuant to division (A)(3) of section 4506.14 of the Revised Code;
  5. (e) A duplicate or reprint commercial driver's license.
2. (2) The registrar or deputy registrar may collect and retain an additional fee equal to twice the amount established under section 4503.038 of the Revised Code for each application received by the registrar or deputy registrar for a commercial driver's license, restricted commercial driver's license, or renewal of such a license that will expire on the licensee's birthday eight years after the date of issuance.
(C)
1. (1) In addition to the fees imposed under divisions (A) and (B) of this section, the registrar of motor vehicles or deputy registrar shall collect the following fee for each application, as applicable:
  1. (a) Twelve dollars for a commercial driver's license temporary instruction permit;
  2. (b) Twelve dollars for a commercial driver's license or restricted commercial driver's license that will expire on the licensee's twenty-first birthday;
  3. (c) Twelve dollars for a commercial driver's license, restricted commercial driver's license, or renewal of such a license that will expire on the licensee's birthday four years after the date of issuance;
  4. (d) Twelve dollars for a commercial driver's license or restricted commercial driver's license issued pursuant to division (A)(3) of section 4506.14 of the Revised Code;
  5. (e) Twelve dollars for a duplicate or reprint commercial driver's license;
  6. (f) Twenty-three dollars and fifty cents for a commercial driver's license, restricted commercial driver's license, or renewal of such a license that will expire on the licensee's birthday eight years after the date of issuance.
2. (2) The additional fee is for the purpose of defraying the department of public safety's costs associated with the administration and enforcement of the motor vehicle and traffic laws of Ohio.
(D) Each deputy registrar shall transmit the fees collected under divisions (A) and (C) of this section in the time and manner prescribed by the registrar. The registrar shall deposit all moneys collected under divisions (A) and (C) of this section into the public safety - highway purposes fund established in section 4501.06 of the Revised Code.
(E) Upon request and payment of a fee of five dollars, the registrar shall furnish information regarding the driving record of any person holding a commercial driver's license issued by this state to the employer or prospective employer of such a person and to any insurer. The registrar shall pay each five-dollar fee the registrar collects under this division into the state treasury to the credit of the public safety - highway purposes fund established in section 4501.06 of the Revised Code.
