Each science, technology, engineering, and mathematics school shall report to the department of education, in the form and manner required by the department, all of the following information:

1. (A) The total number of students enrolled in the school;
2. (B) The number of students who are receiving special education and related services pursuant to an IEP;
3. (C) For each student reported under division (B) of this section, which category specified in divisions (A) to (F) of section 3317.013 of the Revised Code applies to the student;
4. (D) The full-time equivalent number of students who are enrolled in career-technical education programs or classes described in each of divisions (A), (B), (C), (D), and (E) of section 3317.014 of the Revised Code that are provided by the STEM school;
5. (E) The number of students who are limited English proficient students and which category specified in divisions (A) to (C) of section 3317.016 of the Revised Code applies to each student;
6. (F) The number of students reported under division (A) of this section who are economically disadvantaged, as defined by the department. A student shall not be categorically excluded from the number reported under division (F) of this section based on anything other than family income.
7. (G) The resident district of each student;
8. (H) Any additional information the department determines necessary to make payments under this chapter.
