1. (a) in a capacity which can result in contamination of food or food-contact surfaces with disease-causing organisms;
2. (b) while infected with a disease in a communicable form capable of transmission by food;
3. (c) who is otherwise a carrier of organisms that cause such disease; or
4. (d) while afflicted with a boil or infected wounds on hands, face or other exposed part of the body.
