1. (a) Compliance agreements shall be completed at the conclusion of an inspection or at a mutually agreed upon time and place between the license and inspector, with in a time period not to exceed one week.

2. (b) The following shall be provided in the compliance agreement:

   1. (1) The date of the agreement;
   2. (2) The name of the establishment;
   3. (3) The license number of the establishment;
   4. (4) The name of the person in charge of the establishment;
   5. (5) Attestation to the agreement to make repairs, corrections or changes that are necessary to bring the establishment into compliance with He-P 2100;
   6. (6) A list of each repair, correction, or change to be made including the date of intended completion;
   7. (7) The signature of the person in charge;
   8. (8) The title of the signee; and
   9. (9) The signature of the inspector or department representative making the agreement.
3. (c) Failure on the part of the license holder to comply with the signed compliance agreement shall result in action against the license as specified in He-P 2105.01(e) and He-P 2106.02.
