When applying for a license, an applicant shall provide the information required by the department in the form required by the department. The information must include:

1. (1) information on the directors, officers, and controlling persons of the applicant;
2. (2) the applicant's business plan, including at least 10 years of detailed financial projections, and other relevant information; and
3. (3) any additional information considered relevant by the department.
