1. (1) To be found to have a qualifying SDMI diagnosis, an individual must:

   1. (a) be 18 years of age or older;
   2. (b) have a minimum of three areas of high-level impairment indicated by a score of three or higher on the Behavioral Health and Developmental Disabilities (BHDD) Severe and Disabling Mental Illness, Home and Community-Based (HCBS) Waiver, Evaluation and Level of Impairment (LOI) form; and

   3. (c) have experienced one of the following in the previous 12 months:

      1. (i) involuntarily committed because of a mental disorder to the Montana State Hospital or the Montana Mental Health Nursing Care Center for at least 30 consecutive days; or

      2. (ii) diagnosed with one of the following diagnoses, excluding mild or not otherwise specified:

         1. (A) Schizophrenia, paranoid type;
         2. (B) Schizophrenia, disorganized type;
         3. (C) Schizophrenia, catatonic type;
         4. (D) Schizophrenia, undifferentiated type;
         5. (E) Schizophrenia, residual type;
         6. (F) Delusional disorder;
         7. (G) Schizoaffective disorder;
         8. (H) Schizoaffective disorder, depressive type;
         9. (I) Bipolar I disorder, manic, moderate;
         10. (J) Bipolar I disorder, manic, severe without psychotic features;
         11. (K) Bipolar I disorder, manic, severe with psychotic features;
         12. (L) Bipolar I disorder, depressed, moderate;
         13. (M) Bipolar I disorder, depressed, severe without psychotic features;
         14. (N) Bipolar I disorder, depressed, severe with psychotic features;
         15. (O) Bipolar I disorder, mixed, moderate;
         16. (P) Bipolar I disorder, mixed, severe without psychotic features;
         17. (Q) Bipolar I disorder, severe with psychotic features;
         18. (R) Bipolar II disorder;
         19. (S) Major depressive disorder, single, moderate;
         20. (T) Major depressive disorder, single, severe without psychotic features;
         21. (U) Major depressive disorder, single, severe with psychotic features;
         22. (V) Major depressive disorder, recurrent, moderate;
         23. (W) Major depressive disorder, recurrent, severe without psychotic features;
         24. (X) Major depressive disorder, recurrent, severe with psychotic features;
         25. (Y) Post traumatic stress disorder, acute;
         26. (Z) Post traumatic stress disorder, chronic;
         27. (AA) Generalized anxiety disorder; and
         28. (BB) Borderline personality disorder.
